Here is a simple plugin that will do that for you: https://wordpress.org/plugins/tt-sidebar-login-widget

To do it by hand, add a widget in your functions.php like this example: https://codex.wordpress.org/Function_Reference/register_widget

Then put this code to run inside the widget: https://codex.wordpress.org/Function_Reference/wp_login_form
