Hi Mohan :)

Sorry for the delay in getting back to you, after taking a look around for what I think you are asking there is not one specific plugin that looks up to date to create separate pages on your wordpress site bringing in your social feeds.

I think you would be best using specific plugins for each social media group and possibly combining them onto one social media page.

I have picked out some up to date plugins I would try to do this with.

I have not used these plugins before so I would advise reading the docs to get a good understanding of how they work :)
